| HealthShare is in the process
of negotiating and evaluating group health plans and group insurance
options for members. For those who already have satisfactory insurance
coverage, HealthShare is evaluating supplemental plans to cover a mix of
traditional and alternative healthcare. These options will be made available
to HealthShare members as soon as the evaluation is completed.
HealthShare will continue to look for ways to curb and control healthcare costs by seeking whatever savings we can achieve as a group, and by expanding the number of businesses that support HealthShare by providing discounts. Bloomington Chiropractic Center is a comprehensive drugless
and nonsurgical health care clinic. Dr. Houze is a wellness physician utilizing
various complimentary and alternative healing methodologies. At this clinic,

Bloomington, Family Service Association of Monroe County, Inc. offers mental
health counseling for individuals, couples, families and groups. This United
Way agency has served our community for over 30 years! Masters level clinical
social workers and counselors provide the services along with a consulting
psychiatrist. Parking is available. Family Service Association has become
the latest Consumer Club provider, offering discounted counseling services
to HealthShare members. Members will receive a discount of 30% off the
regular counseling fee of $75.00 per hour. Members must make payment at
time of service. Services rendered by appointment only. You must present

| ConvaCare Services, Inc. is a privately owned and operated distributor
of home medical equipment and services with its headquarters located in
Bedford, Indiana. ConvaCare operates through a network of independent pharmacies
and branch offices. ConvaCare maintains an inventory of medical equipment
and supplies that satisfies most patient needs. Special equipment or product
needs are available for overnight shipment to the Partner nearest you.
The basic home medical equipment product line includes wheelchairs, walking
& mobility aids, hospital beds, traction equipment, patient care equipment,
rehabilitation & physical therapy equipment, transcutaneous electrical
nerve stimulators (TENS), oxygen & respiratory therapy equipment, apnea
monitors, and more. Most Partners also carry bathroom aids, home diagnostic
kits, aids to daily living (ADLs), patient comfort & convenience items,
incontinence & urinary care products, orthopedic supplies, ostomy products,
and more. ConvaCare files third-party health insurance claims on behalf
of the patient. They also service rental equipment and provide warranty
repairs, including routine maintenance for privately owned equipment. In
many cases, ConvaCare will provide a substitute until your equipment is
ready. Consumer Club members receive a discount of thirty percent (30%)
off the prices at which services are normally charged. The discount applies
for those services which are not insurance reimbursable. (This means that
all insurance copays are ineligible for this discount.) Payment is expected

drive-through convenience. Anthony Catozzi, the only independent pharmacist
in Bloomington, owns and operates Pharmacy1 Express. HealthShare performed
extensive cost comparisons before approaching him to be a discount vendor
and found he already had some of the lowest prices in town! Pharmacy1 Express
sells prescription drugs to members at a substantial discount. Call ahead
for price comparisons. When you pick up your low-cost prescriptions at
Pharmacy1 Express, don't forget to present your HealthShare membership
card! The Consumer Club discount is not valid when used in conjunction
with another prescription drug program or discount (i.e., insurance copay).
